Job Description
  • Contractor will be responsible for the planning, development, and administration of a statewide project, Rural Mobile Family Medicine.
  • The Rural Mobile Family Medicine project will be a system of mobile family medicine services and outreach for well-visits, screenings, and chronic disease management, with telehealth support for rural residents across the lifespan.
Responsibilities
  • Plan, organize, direct and administrator a large public health project, Rural Mobile Family Medicine.
  • Plan, assign, coordinate, oversee, and review the work of professionals, subject matter experts, and technical consultants engaged in project development, implementation, and evaluation.
  • Work with other state agencies and organizations to ensure cross-system collaboration and implement integrated services and policies related to Rural Health Transformation Program components and non-RHTP initiatives and policies.
  • Establish and convene Rural Mobile Family Medicine committee on an ongoing basis to review formative research, design initiative, and ensure ongoing coordination between subject matter experts and committee for development, implementation, and evaluation.
  • Oversee formative research, policies, budget, evaluation and performance measurement and identify and use public health data and evaluation findings as a tool to develop and prioritize all aspects of Rural Mobile Family Medicine.
  • Establish Rural Mobile Family Medicine in a timely manner based on a thorough assessment of barriers, needs, weaknesses, and strengths of family medicine services within rural communities.
  • Secure contract(s) and provide oversight and monitoring of contractors to conduct assessments.
  • Secure and oversee contract(s) with subject matter expert(s) to complete Rural Mobile Family Medicine plan and design, including healthcare services provided, structure, implementation plan and timeline, and complex program analysis and evaluation; development; and implementation; as well as other tasks as applicable.
  • Provide day-to-day grant management to ensure maximum efficiency and effectiveness, including:
    • Ongoing communication 
    • Budget, contract, and invoice management
    • Completion of grant deliverables (e.g., continuation applications, budgets, reporting to federal funders (EMS).
  • Oversight of at least one Rural Mobile Family Medicine project specialist that will be responsible for the implementation of the Rural Mobile Family Medicine
  • Provide technical assistance to partners, subcontractors, and others as needed
  • Support the planning, information gathering, scheduling, logistics, resource management, task execution, progress tracking, documentation, and stakeholder communication necessary to implement the Rural Mobile Family Medicine initiative.
  • Balance needs, requirements, partnership, and workload for the multi-faceted projects within initiative.
  • Prepare extensive technical and statistical written reports related to the program’s activities.
  • Attend and present at state, local, and national meetings and conferences involving state, federal and local officials, professionals, and the public concerning development, implementation, and evaluation of Rural Mobile Family Medicine.
  • Perform other duties as assigned.
Education, Experience and Required Skills
  • Master’s degree or higher preferred.
  • Minimum of five years of experience with oversight of a multi-faceted initiative or large program within the healthcare system to include research, policy development planning, administration, and evaluation.
  • Experience working with mobile health medicine and community-based healthcare.
  • Strong budget and contract management skills.
  • Strong written and oral communications skills.
  • Strong project management skills.
