Developing a New Care Coordination Platform on Salesforce Health Cloud for a Leading Health Care Provider

Our client, the world's leading provider of dialysis products and services, supports millions of patients living with chronic kidney disease (CKD), including approximately 3.5 million individuals worldwide who rely on dialysis treatment.

PROJECT

The client set out to enhance care coordination for patients with chronic kidney disorder by developing an integrated Patient and Care Team platform. Their objective was to improve communication among care providers while delivering a more seamless, patient-centered experience. A key element of this initiative was the integration of in-home dialysis treatment within the broader care ecosystem.



The platform was designed to provide home dialysis patients with a single, centralized location to manage all aspects of their treatment—from ordering supplies to accessing clinical data. In parallel, the client expanded the platform to include Home Nurse functionality, supporting nursing workflows and day-to-day order management to enable more efficient, coordinated care delivery.

CHALLENGE

The client required a large, highly experienced team of Salesforce Developers, Business Systems Analysts, Testers, and Project Managers to design and build HealthCloud applications. These were long-term engagements, with many roles extending or converting to permanent positions, and required on-site support.



Senior Developers needed deep expertise in Salesforce Sales Cloud and Service Cloud, as well as Salesforce web services APIs and Force.com, including Lightning Single Page Architecture and design systems. Project Managers were required to hold PMP and CSM certifications, while Business Systems Analysts needed strong clinical systems experience, a deep understanding of the healthcare industry, and the ability to connect complex requirements, solve problems, and anticipate future needs.

APPROACH

We partnered closely with the Clinical IS development team leaders and the PMO to define a targeted strategy for identifying the most qualified consultants for the engagement. Together, we aligned on the technical requirements, experience levels, and cultural attributes needed to ensure a strong fit within the organization.


To validate technical expertise, our recruiting team leveraged a customized screening process along with additional vetting techniques to assess high-quality, qualified candidates. We also implemented a streamlined interview process, conducting multiple rounds of pre-screening on our side to help the client quickly and confidently select candidates who were well-suited for their teams.

IMPACT

Talent Groups delivered a full spectrum of IT talent to support the development initiative, successfully placing 10 Senior Developers, 2 Salesforce QA specialists, 9 Business Systems Analysts, and 3 Project Managers.


With the right teams in place, the client launched multiple phases of the project, resulting in measurable improvements to patient experience and enhanced care coordination across the organization.
